How student(s) can request a preferred name change.

If you are a student who would like your name changed in Canvas you can fill-out the Preferred Name form found on the Admissions Form site. You may submit the Preferred Name Form at any time. Your request will take effect within seven (7) business days of receipt of the Preferred Name Form.

Please note: your preferred name will take effect at the beginning of the following term.
Government issued picture ID is required when submitting this form in person.
